With helping hands and open hearts, Hospice of Southwest Ohio provides care, comfort and compassion for those facing life-limiting illnesses. We’re a locally managed team of dedicated nurses, caregivers and volunteers whose main goal is to keep your loved ones at peace, in a place full of care and compassion.

Hospice of Southwest Ohio serves five counties, with the highest reputation for treating patients with the care, dignity and the respect everyone deserves. We do it with minimal or no cost to you and your family.

Tabetha Morelock, STNA

Tabetha Marie Morelock was born in Stuttgart, Germany, and moved to the states when just six months old. Tabetha graduated from Hamilton High School and immediately became a State Tested Nursing Assistant. Tabetha has been with Hospice of Southwest Ohio for five years and enjoys the quality one-on-one time that she gets to share with her patients. This time is very important for her to have the ability to care for people who are unable to do much for themselves.


Jane Keller, Admissions

I have a job that I love with Hospice of Southwest Ohio as an Admissions Coordinator. I also enjoy being in the field as a nurse making second visits to our clients. I have been a nurse for almost 20 years, holding various positions in the hospice field the majority of those years. I never imagined myself in this field and cannot imagine myself ever leaving it.


Tracy Paul Downs, Chaplain

Tracy Paul Downs has spent over 11 years with Hospice of Southwest Ohio. “I love our company. It has been an awesome privilege to see it go from a folding table and a phone to what it is now. How awesome it has been to be a part of the growth and development of this company. I truly pray that this is the only job that I ever have! I love the job because we have the opportunity to meet people right where they are…in the moment. We get to move beyond surface issues and really reach out to people on a very deep and spiritual level. We have such a unique opportunity to care for others in a very profound and meaningful way. We also get to observe people at their very best, as we see others care for their loved ones and give of themselves. I am truly inspired by the family caregivers that we minister to on a daily basis. I am encouraged by their strength and courage and by their willingness to let us into their lives.

Your Care Team

Our interdisciplinary care team works together to support every aspect of an individual’s medical, spiritual and emotional care. We provide care for patients in private homes, in nursing homes and in Hospice of Southwest Ohio’s inpatient Patient Care Center in Madeira.


Hospice of Southwest Ohio has a medical director who leads our team of physicians and consults with attending physicians when needed. Each of our physicians works with an individual’s personal physician and helps to direct the rest of the care team.


Our nurses are specially trained in hospice care and excel at symptom management. They are available 24 hours a day to answer questions or address symptom needs.

Nurses initiate a plan of care under a physician’s orders and update that plan each time a patient is seen. For patients outside our Patient Care Center, our nurses visit an average of one to two times a week, with flexibility to increase visits to provide optimal care.

Social Workers

Our social workers provide emotional support and counsel to both the family and the patient. Each social worker can help secure additional community resources, provide assistance with insurance issues and arrange volunteer support. Our social workers are trained in the hospice process and how to provide the proper support to loved ones and their families.


If spiritual support is requested, our chaplains can provide counsel to both the patient and family. The chaplain will also offer assistance with funeral planning and officiate at a funeral service, as needed. Our support continues with up to 13 months of bereavement counseling after a patient’s death.

Home Health Aides

Our aides provide the day-to-day personal care requested in the patient’s plan of care and act as the liaison between the hospice team and the family. Aides can help an individual with personal grooming, light housekeeping, meal preparation and more. Our aides also offer caregivers a break from their daily routine.


We are proud of our hospice volunteers and their willingness to support those in need. Some of our volunteers provide companionship to patients and their families, including running errands, reading to patients or delivering medications. Other volunteers support hospice by handling administrative duties for our hospice staff.
