Your Care Team

Our interdisciplinary care team works together to support every aspect of an individual’s medical, spiritual and emotional care.

Registered Nurse

Registered Nurse | Hopsice of Southwest OhioA registered nurse will access the patient within 24 hours of admission. The nurse is the case manager for the patient/family and coordinates personalized care with our entire team. A nurse will make visits to monitor and evaluate an individual’s condition and educate the family and team on palliative care options.

Home Health Aides

Home Health Aids | Hopsice of Southwest OhioOur aides provide the day-to-day personal care requested in the Plan of Care (POC) and act as the liaison between the hospice team and the family. Aides can help an individual with personal grooming, light housekeeping, meal preparation and more. Our aides can also offer caregivers a break from their daily routine.

Social Worker

Social Worker | Hopsice of Southwest OhioOur social workers provide emotional support and counsel to both the family and the patient. Each social worker can help secure additional community resources, provide assistance with insurance issues and arrange volunteer support. Our social workers are trained in the hospice process and how to provide the proper support to loved ones and their families.

Chaplain

Chaplain | Hopsice of Southwest OhioIf spiritual support is requested, our chaplains can provide counsel to both the patient and family. The chaplain will also offer assistance with funeral planning and officiate at a funeral service, as needed. Our support continues with up to 13 months of bereavement counseling after a patient’s death.

Esteemed Volunteers

Volunteers | Hopsice of Southwest OhioWe are proud of our esteemed hospice volunteers and their willingness to support those in need. Some of our volunteers provide companionship to patients and their families, including running errands, reading to patients or delivering medications. Other volunteers support hospice by handling administrative duties for our hospice staff.

Physician

Physician | Hopsice of Southwest OhioHospice of Southwest Ohio has a medical director who leads our team of physicians and consults with attending physicians when needed. Our physician works with an individual’s personal physician and helps to direct the rest of the care team.
